Health Care Assistant- B3

We are currently recruiting for experienced Band 3 care support assistants to work on a community project covering Harlow, Epping and Uttlesford. The service is committed to delivering high quality care using a multi disciplinary approach. The post holder will be responsible for re-ablement to help patients who have been discharged from hospital or who are requiring assistance to remain living at home to regain their independence, manage their long-term conditions and support patients during periods of personal crisis. The post holder will be expected to work without direct supervision for all or most of the shift within patients' homes. You must be a car driver and have access to your own vehicle.

Duties of the position:

To carry out delegated work as set out in the patient's care plan initiated by the co-coordinator.

Support with patients' rehabilitation on returning home from a stay in hospital.

Pro active and responsive support to prevent patients' admission or re-admission into hospital.

To only undertake tasks the post holder feels competent to perform as directed by the Team Lead or Care Coordinators.

To work unsupervised, using own initiative as required.

To be adaptable and confident to deal with unexpected situations.

To prioritise workload.

To always act in a professional and confident way.

What we need from you:

Care experience within Hospitals/ community or Care Home essential

NVQ 2 or 3 desirable but not essential

Willing to complete required agency compliance

Hold a current driving license with access to a car
