We are currently recruiting for a community-based project based in Buckinghamshire. The service offers support for progressive life-limiting illness for patients that live in the areas of Buckinghamshire. The service will support patients to live the best quality of life that they can through the combination supportive care services at Home. The patients have a range of illnesses which include, but are not limited to, cancer, neurological conditions, heart failure, organ failure and chronic lung disease.

We are recruiting immediately to take on a team of dedicated and experienced Health Care Professionals to work in patients' homes providing crucial end of life care. This will include personal care, prompting medication, physical and emotional support for each patient within the comfort of their own home.

The project currently covers patients' homes across Buckinghamshire in the following locations (you must be willing to travel to any of the locations below):

Gerrards Cross

Marlow

Princes Risborough

High Wycombe

Chinnor

The rota for this project runs from Monday to Sunday and is mostly long day shifts (times below) however do vary day to day and hours could be reduced:

08:00-20:00

09:00-21:00

Requirements:

You must have a full UK Driving Licence and have access to your own car

You must have a DBS or be willing to get one

You must be reliable, punctual, and compassionate
